- Conduct comprehensive health assessments, including medical history reviews, physical examinations, and vital sign measurements, to identify patient health concerns and risk factors.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers, focusing on preventive care, health promotion, and chronic disease management.
- Educate patients and families about health conditions, treatment options, medications, lifestyle modifications, and preventive measures to empower them in self-management and decision-making.
- Administer medications, vaccinations, and treatments as prescribed by healthcare providers, ensuring accuracy, safety, and adherence to established protocols.
- Perform various clinical procedures and interventions, such as wound care, injections, specimen collection, and diagnostic tests, following standard procedures and infection control guidelines.
- Monitor patients' progress, review laboratory and diagnostic test results, and evaluate treatment effectiveness, collaborating with healthcare team members to modify care plans as needed.
- Provide counseling, emotional support, and referrals to patients and families, addressing their psychosocial, emotional, and spiritual needs throughout the healthcare continuum.
- Document patient assessments, interventions, and outcomes accurately and comprehensively in electronic health records (EHR) systems, maintaining confidentiality and compliance with regulatory standards.
- Coordinate care transitions, referrals to specialty services, and follow-up appointments, ensuring continuity of care and effective communication among healthcare providers and community resources.
- Participate in quality improvement initiatives, evidence-based practice projects, and professional development activities to enhance clinical competencies and promote excellence in patient care.
- A Bachelor's Degree in Nursing is required.
- Candidates must possess a minimum of 5 years of experience in Clinical Nursing.
- 2 years experience in Primary/Outpatient Care.
- Must be eligible to take the SCFHS exam.
- Preferably with valid SCFHS Nursing Specialist/Technician license/certificate.
- Current license as a professional registered nurse.
- Proficient in English
